Redding Rancheria is seeking a RN Case Manager. The primary function of the RN Case Manager is to provide case management services for patients while accessing and linking patients to resources in the community. The RN Case Manager is responsible for making appropriate referrals, appointment setting, and monitoring/reporting patients’ progress and needs to the clinical team. The RN Case Manager will also be responsible for documenting case notes, and entering data regarding patients. Assists with clinical nursing duties as needed. Reports directly to primary clinician, and nursing supervisor, any change in status which requires treatment intervention.
Provides professional nursing services to accomplish the individual patient plan of care; independently and consistently using the nursing process to safely, therapeutically, and efficiently care for a full assignment of patients with subtle and sometimes unidentified needs.
- Graduate of accredited school of registered nursing and possess current/valid R.N. license.
- BSN preferred.
- CPR certification.
- Knowledge of EHR systems, Next Gen preferred.
